The Schenectady County Public Health Services will offer a clinic to provide the H1N1 vaccine for free to students currently enrolled in Scotia-Glenville programs (all students enrolled in Scotia-Glenville Schools from Kindergarten to Grade 12) as well as to students who attend school outside of the school district boundaries or who are home-schooled. This clinic will be set up at Sacandaga School on Wednesday, December 16. There will be arrangements made to bring students participating in the clinic from Scotia-Glenville's district schools to this clinic throughout the day. Parents of private school and home-school students are responsible for bringing their child to the Sacandaga School between 7:45 a.m. and 2:45 a.m. on December 16 for the vaccine. A school-based clinic was held at the Lincoln Elementary School on Nov. 17. Below is the paperwork, in PDF format, that has been mailed to all students: |
| Consent form to receive the H1N1 vaccine. This form has to be completed as a single page back-to-back document. Instead of downloading and completing this form as two pages, there will be two-sided copies at Sacandaga that can be completed. Letter from the Schenectady County Public Health Services to parents regarding the H1N1 vaccine. |
