Please be sure you keep your child home from school if he or she has a high fever, a cough and exhibits flu-like symptoms.
As well, teach your child to cover his or her mouths when sneezing or coughing and to wash his or her hands frequently.

Schenectady County Public Health Services H1N1 (swine flu) clinic for all school-age children residing in the Scotia-Glenville School District to be held on Wednesday, December 16, at the Sacandaga School.
Advanced registration is required.
Paperwork should be returned to your child's school by Friday, Dec. 11.

REQUIRED HEALTH FORMS

Physical Form

All students entering grades 2, 4, 7 and 10, as well as ALL new students entering any grade, must have a physical examination.

The exam is considered valid if the date of the exam is within ONE YEAR prior to the start of the school year in which the physical exam is required.

Medical Certificate of Physical Limitations

Should a student be temporarily or permanently unable to participate
in the regular program of physical education, a program must be developed to meet the needs of
that student. A certified adaptive physical education teacher is on staff.

Dental Certificate

New York State requires districts to request a dental health certificate from parents as part of the general health assessment (Chapter 281 of the Laws of 2007) for children entering grades 2, 4, 7 and 10.
